In order to provide high quality care and to maintain competitive rates, we no longer accept insurance. We can provide a receipt with the information needed for you to submit it to your insurance company. Services may be covered in full or in part by your health insurance or employee benefit plan. Please verify with your insurance company prior to scheduling appointments to confirm that they will reimburse you and if you have a deductible. You are responsible for payment whether or not your insurance company reimburses you.
Payment is due at the time of appointment. We accept cash and check.
